Responsibilities

• Assist patients with activities of daily living, including bathing, dressing, grooming, toileting, feeding, and mobility.
• Obtain and document vital signs, intake and output, weights, and other assigned observations.
• Absorb and document changes in patient condition to the licensed nurse.
• Maintain accurate and timely documentation of patient care.
• Ensure a clean, safe, and comfortable environment while following infection prevention and safety standards.
• Collaborate with nurses and the interdisciplinary healthcare team to support quality patient care.
• Maintain patient confidentiality and comply with HIPAA and facility policies.
• Perform other duties as assigned.

Requirements

• A High School Diploma or equivalent is preferred.
• Current Certified Nursing Assistant (CNA) certification in the state of practice.
• Recent experience in long-term care (LTC), skilled nursing (SNF), rehabilitation, hospital, or acute care is preferred.
• Current BLS certification preferred or required per facility policy.
