About the role

Provides direct patient care to residents in a long-term care setting under the supervision of a licensed nurse. Assists with activities of daily living (ADLs), monitors patient well-being, maintains accurate documentation, and supports a safe, clean, and comfortable environment while delivering compassionate, resident-centered care.

Responsibilities

  • Assists with bathing, dressing, grooming, toileting, feeding, transferring, and other activities of daily living.
  • Measures and documents vital signs, intake/output, and patient observations as directed.
  • Makes observations of residents for changes in condition and reports concerns to the nursing staff promptly.
  • Maintains accurate and timely patient documentation in the electronic medical record.
  • Aids in patient mobility using proper safety techniques and equipment.
  • Maintains a clean, safe, and organized patient environment while following infection control policies.
  • Provides compassionate, respectful, and resident-centered care while protecting patient dignity and privacy.
  • Communicates effectively with nurses, physicians, therapists, residents, and family members.
  • Performs other job-related duties as assigned.

Requirements

  • Active Certified Nursing Assistant (CNA) certification in the state of practice.
  • Current BLS certification (if required by facility).
  • Long-term care (LTC), skilled nursing facility (SNF), or rehabilitation experience preferred.
  • Ability to work evening or night shifts, weekends, and holidays as required.
  • Experience with PCC charting or willingness to learn preferred.
  • Excellent communication, teamwork, and patient care skills.
