Key Responsibilities

• Attends to the daily living and safety needs of an assigned group of patients.
• Affixes assistance to registered nursing personnel in the care of patients by performing various patient care activities and related non-professional services.
• Collaborates and coordinates with other departments to provide timely effective care consistent with individual's needs, choices, and preferences.
• Obtains and records vital signs, heights, and weights per policy.
• Maintains patient's personal hygiene and basic needs in a timely manner.
• Ensures that the patient is comfortable, necessary supplies are available and in reach.
• Keeps patient environment safe and tidy.
• Provides patient-specific care as appropriate to age of patient.
• Demonstrates effective communication skills with children, adolescents, adults, and geriatric patients to include use of appropriate therapies such as play, written, and picture board.
• Reports changes in patient's condition, out-of-normal values, patient/family concerns or complaints to nurse, charge nurse, and/or supervisor.
• Applies AIDET as well as other evidence-based behaviors.
• Participates in developmental and organizational initiatives for process improvement and quality assurance as requested.
• Follows appropriate infection control procedures.
• Performs skin care interventions and treatments, as permitted by state regulation.
• Maintains patient and staff confidentiality.
• Assists in the orientation of new staff as requested.

Unit Specific Responsibilities

• Under the direction of a registered nurse (RN), provides continuous visual and proximal contact with the patient(s), including but not limited to in-room observation, provision of basic comfort services, maintaining a safe environment, accompanying patient during transportation for tests/procedures, and documentation on appropriate forms.
• Maintains educational requirements and training requirements to fulfill the role and responsibilities of a Unit Coordinator as outlined in the Unit Coordinator job description.
• Regularly updates patient communication boards.
• Performs hourly rounds with patients.
• Performs bedside shift report.
• Assures patient belongings are accounted for on admission and transfers and returned at discharge.
• Performs designated admitting tasks.
• Records accurate intake and output for each patient.
• Performs designated admitting tasks.

Home Health and Hospice

• All documentation is completed accurately and timely (no later than 8am the next day).
• Productivity to average no less than 5-6 patients per day.
• Performs hourly rounds with patients.
• Performs bedside shift report.
• Assures patient belongings are accounted for on admission and transfers and returned at discharge.
• Records accurate intake and output for each patient.
• Performs designated admitting tasks.

Qualifications

• Must be 16 years of age or greater, if CNA; Must be 18 years of age or greater, if LNA.
• Education: High school diploma or equivalent, if providing POC Testing.
• Experience: Previous experience as a Nursing Assistant, OR successful completion of a Nursing Assistant certification program.
• Licensure: N/A Certification: Current and valid Certified Nursing Assistant Certification, if CNA; Current BLS through the American Heart Association or American Red Cross.
• Pediatrics Unit Specific: Two-week pediatric orientation with annual competency in pediatrics.
• Home Health and Hospice: Level one fingerprint clearance card, or application, required within 20 days of hire; Current and valid Arizona Driver’s License, must be 21 years of age or older and clean driving record; Current and valid Arizona Food Handlers Card.
• Preferences: Experience as an LNA or CNA in related unit’s field; High School Diploma or GED equivalent.

Special Position Requirements

• Exposure Categories: Category I: Expected duties have routine potential for exposure to blood, body fluids or tissues.
• Home Health/Hospice: Requires travel, in properly insured personal vehicle, throughout the service area.

Work Requirements

• Able to distinguish appropriate colors, if providing POC testing.
• Able to handle multiple priorities simultaneously.
• Demonstrates good oral and written communication skills.
• Able to assist with the movement of an adult patient weighing up to 300 lbs.
• Able to stand, bend, stoop, kneel, and reach, while working with patients for the entire shift.
• Able to care for patients with infectious diseases.
• Able to sit for 3-5 hours per day.
• Able to stand for 3-5 hours per day.
• Able to walk for 3-5 hours day.
• Proficiently utilizes office equipment including phone, computer, fax printers, copier, scanner, etc.
