CNA- Life Services Coordinator

Responsibilities

• Provides services to tenants as assigned and as indicated on Resident Service Plans, which may include grooming and dressing guidance, assistance with bathing, and medication reminders/ assistance.
• Reports any significant tenant changes to the nurse.
• Monitors for changes in tenant needs, preferences, and/or health status, and reports/documents any changes according to established procedures.
• Reviews the Staff 24-Hour Communication log and corresponding Tenant Service Notes on a daily basis for information about changes in tenant service needs and/or health conditions.
• Prepares and serves meals/snacks, monitors food expiration dates and maintains kitchen cleanliness.
• Completes various housekeeping duties including: dusting, vacuuming, cleaning bathrooms and common spaces, maintaining kitchen cleanliness, make and change bed linens, and laundry services.
• Maintains activity calendar and engages tenants in activities such as spiritual, educational, games, music, and physical activities.
• Provides companionship.
• Works effectively with minimal supervision and in a team setting.
• Follows safety policies in performing care, assuring resident safety.
• Follows and upholds policies and procedures.
• Attends and participates in facility meetings and in-service training programs as assigned.
• Follows infection control guidelines.
• Observes residents’ rights including, but not limited to, privacy, dignity, etc.

Qualifications

The ideal candidate will possess a valid state certification as a Certified Nursing Assistant (CNA).

Skills

Strong interpersonal skills, ability to work independently, and a positive attitude are essential.

Schedule

• Shifts: In general, we offer 8-hour shifts for our CNA positions. We have openings for full-time (30+ hours weekly) and part-time (up to 29 hours weekly) positions.
• Days: 6am-2:30 pm
• Evenings: 2pm-10:30pm
• Nights: 10pm-6:30am
