Brief Description
Under the supervision of the Hospice Registered Nurse, the CNA Hospice Aide/Homemaker provides personal care and light housekeeping services to terminally ill patients in various settings such as the home, nursing facility, inpatient units, or other environments where the Hospice patient resides. The Aide actively participates as a member of the Hospice Interdisciplinary Team (IDT), providing care and comfort to patients according to their assigned tasks and the interdisciplinary plan of care. They observe the patient, report observations, and document care performed. Documentation of services is timely based on the patient's care plan.
Education
- High school diploma or equivalent required.
- Completion of an approved hospice aide training program as outlined under the Hospice Conditions of Participation 418.76: CNA Hospice Aide/Homemaker Services.
- Certification as a Hospice Aide from the program.
- Completion of a nurse aide training and competency evaluation program approved by the State as meeting the requirements under Medicare 483.151 through 483.154, and being listed in good standing on the State nurse aide registry.
- Completion of a state-approved training program and certification as a home health aide, with proof of certification.
- Demonstration of skills and passing a written competency test prior to working independently with a patient.
Experience
- Must be at least 18 years old.
- One (1) year of direct patient care experience in an institutional or home care setting required, with community health/hospice experience preferred.
- Understanding of Hospice philosophy and needs of terminally ill.
Special Employment Requirements
- Clearance by PA State Police criminal history check.
- Professional attitude and demeanor, effective communication, and cooperative behavior with patients, caregivers, colleagues, supervisors, co-workers, and all others.
- Valid PA driver’s license and availability for transportation during all working hours.

Skills
- Visual/hearing ability sufficient for comprehension of written/verbal communication.
- Able to effectively manage stress.
- Manual dexterity sufficient for writing, keyboarding, and telephone operation.
