FLSA Status: Non-Exempt. Schedule: Monday–Friday, 8a-5p. Location: Plymouth. Travel is required.

Responsibilities
- Travel to where the patient lives and calls home
- Provide care and services to patients according to the care plan or as approved by an RN
- Assist with activities of daily living as directed by an RN
- Offer support and encouragement to patients and families
- Function within acceptable time frames for daily patient visits
- Clearly communicate information regarding changes in the patient’s mental/physical condition, complaints, or special requests to the Agency nurse and ECF unit manager if applicable
- Demonstrate good judgment in knowing what information needs to be communicated and handle communication in a timely manner
- Maintain documentation of patient care on both Agency forms and the ECF record if applicable, using the appropriate Mobile Care documentation module or paper forms
- Record information accurately, thoroughly, and in a timely manner
- Commit to Agency resolution against fraud and abuse
- Demonstrate knowledge of hospice philosophy and state and federal rules and regulations
- Respect confidentiality of patients/families and other Agency employees
- Attend educational meetings/conferences for professional growth as required
- Follow directions and policies that allow for working under time pressures without sacrificing quality
Requirements
- Must be able to read and write English proficiently
- Must be a state-tested nursing assistant in good standing
- Prior training and experience as a Home Health Aide with Indiana certification as a Nurse Aide
- Must pass drug screen and background check
- Compliance with Annual Flu Shot and Covid Vaccination Policy or ability to provide exemption documentation
- Pass TB test
- Provide an active auto insurance policy as a licensed driver in the State of Indiana with the proper level of coverage as directed
- Provide an active driver’s license
- Ability to provide proof of eligibility to work in the United States
- High level of integrity, ethics, and professionalism
