Responsibilities

• Serve as a clinical leader, delivering high-quality, personalized nursing care directly in patients' homes.
• Develop, implement, and dynamically adjust comprehensive care plans that promote patient independence and well-being.
• Collaborate and coordinate with a multidisciplinary team—physicians, therapists, and support staff—to ensure holistic care.
• Empower and educate patients and their families, providing crucial teaching and emotional support.
• Mentor and guide LPNs, LVNs, and Home Health Aides to foster a strong, cohesive care team.
• Utilize leading-edge tools and best practices to optimize care delivery and maintain precise documentation.
• Oversee your caseload with a 1:1 patient-clinician ratio, ensuring undivided time and attention for each patient.
• Guide patients through the full healing process, from initial visit to recovery, adapting care plans and celebrating milestones.

Requirements

• Graduate of an accredited nursing program with an active RN license in the state served.
• Minimum of 1 year of nursing experience (Home Health experience is a significant plus).
• Exceptional critical thinking, proven clinical expertise, strong case management abilities, and an unwavering commitment to patient-centered care.
• Current CPR/BLS certification.
• Valid driver’s license and reliable transportation.

Pay

$37.40 - $55.63 per hour.
