About the role

This position assists patients with activities of daily living and personal care, collaborating with the patient, family, and healthcare team.

Responsibilities

  • Provides personal and hygienic care as per the care plan and basic care standards.
  • Maintains a clean, safe, and comfortable environment.
  • Ensures privacy and respects patient’s dignity and property.
  • Completes assignments in a timely manner.
  • Follows the plan of care assigned by the Registered Nurse.
  • Relays critical information to the appropriate staff.
  • Confers with the primary nurse on the original plan of care and updates.
  • Documents patient status and care provided.
  • Reports observed changes in patient condition and abnormalities in vital signs.
  • Performs treatments and procedures according to standards.
  • Identifies the stages of death and dying.

Requirements

  • High school diploma or equivalent.
  • Certification issued by state where services will be performed (MD: Certified Nursing Assistant, OH: Nurse's Aide, PA: Nurse's Aide, WV: Nurse's Aide Certification).
  • Valid driver’s license.
  • CPR Certification required within 30 days of hire date.

Qualifications

  • Physical demands: Frequent walking, standing, stooping, kneeling, reaching, pushing, pulling, lifting, and grasping.
  • Physical environment: Exposure to bodily fluids, human tissues, contagious diseases, sharps, toxic gases, fumes, odors, adverse or unpleasant home environments, and unpredictable situations.

Skills and Abilities

  • Concentrated and complex mental activity.
  • Work under highly-stressful conditions.
  • Make sound, independent judgments and collaborate with other multi-disciplinary team members.
  • Comprehend and perform oral and written instructions and procedures.
  • Strong communication skills, both written and verbal.
  • Basic math skills.
  • Ability to work weekends, holidays, and all shifts.
  • Operate telephone, fax machine, beepers, and computer appropriately.
  • Read and write legibly.
  • Knowledge of safety policies and procedures.
  • Knowledge of specialized needs of the terminally ill patient.
  • Knowledge of compliance with Federal, State, and Local regulations/laws.
