About the role

Provides personal care, light housekeeping, and assistance with Activities of Daily Living (ADLs) as outlined in each participant’s plan of care in participant homes and at the PACE center. Exercises independent judgment, reports changes in participant status to the Interdisciplinary Team (IDT), and participates in infection control and cleaning tasks in accordance with CDC guidelines.

Responsibilities

• Under the supervision of the RN, assists with ADL needs of participants both at the PACE center and in participants’ homes (e.g., community, Assisted Living Facilities, Rest Homes, Supportive Housing programs).
• Contributes to the development of a care plan for participants through interaction with the Interdisciplinary Team.
• Collaborates with the Interdisciplinary Team to ensure appropriateness and continuity of care.
• Carries out non-skilled treatments, including vital signs, transfers, toileting, bathing, and dressing at the PACE Center and in the community.
• Assists with individual and group programs by helping plan treatment programs, motivating participants, and evaluating program effectiveness.
• Assists registered therapists and certified therapy assistants with participant-specific activities, such as positioning, transfers, ambulation, and exercises.
• Maintains a clean and safe environment in the PACE center and participants’ homes.
• Assists with meal and snack preparation, serving, feeding, and clean-up.
• Uses safe techniques in all interactions with participants.
• Provides accurate and timely documentation in the EMR and other systems as required.
• Participates in daily cleaning and disinfecting of the PACE center in accordance with CDC guidelines.
• Actively participates in distributing work for health aides to ensure participant care needs and site tasks are completed.
• Attends regular team meetings for health aides at the respective site.
• Performs all duties in accordance with Fallon Health and Summit ElderCare policies and procedures.

Requirements

• Completion of an approved Home Health Aide or Certified Nursing Assistant Training Program is highly desirable, but equivalent work experience may be accepted in lieu of certification.
• Certification as a Home Health Aide or Nursing Assistant is desired but not required if equivalent experience is demonstrated.
• Reliable transportation is required.
• One year of experience working with a frail and/or older adult population, preferably in a PACE program. If under one year, must complete Summit ElderCare training program for health aides prior to working independently.
• Must complete a standardized set of Summit ElderCare competencies for Health Aides on an annual basis.

Pay

$21.50 per hour weekdays and $24.00 per hour weekends. Mileage reimbursement is provided.
