About the Role

Provide routine daily nursing care and services to assigned residents in accordance with each resident's assessment and care plan, under the direction of supervisors.

Responsibilities

• Identify residents using wristband or photo card file before administering treatments or serving meals.
• Use only authorized abbreviations when recording information.
• Report all changes in resident’s condition to the Nurse Supervisor/Charge Nurse promptly.
• Record entries on flow sheets, notes, and charts in an informative and descriptive manner.
• Report all accidents and incidents observed during the shift.
• Maintain confidentiality of resident’s protected health information; report suspected violations.
• Report unauthorized attempts to access the facility’s information system.
• Perform only nursing care procedures for which you have been trained.
• Prepare resident’s room for admission (bed made, name tags up, admission kit available).
• Greet residents, escort them to their room, and introduce them to roommates and staff.
• Inventory and mark resident’s personal possessions; assist with packing/unpacking during transfers or discharges.
• Transport residents to new rooms or receiving areas; assist with loading/unloading residents to/from vehicles.
• Maintain a warm, calm, and positive environment for residents.
• Meet regularly with nursing personnel to identify and correct problem areas or improve services.
• Report all resident complaints and grievances.
• Participate in and receive nursing reports as instructed.
• Follow policies concerning exposure to blood/body fluids.
• Make beds, measure and record vital signs (temperatures, pulses, respirations), and weigh residents as instructed.
• Check on residents unable to call for help frequently; answer resident calls promptly.
• Assist residents with meals (e.g., identifying food arrangements, recording food/fluid intake).
• Keep residents’ water pitchers clean, filled, and within reach.
• Perform after-meal care (e.g., remove trays, clean residents’ hands, face, and clothing).
• Perform all assigned tasks in accordance with established policies and procedures.
• Follow work assignments and schedules to complete tasks.
• Cooperate with inter-departmental personnel to maintain nursing services.
• Notify the facility of lateness or absence from work.
• Report occupational exposures to blood, body fluids, or hazardous chemicals per facility policies.
• Assist residents with daily functions (dental/mouth care, bathing, dressing, combing hair).
• Keep residents dry by changing gowns, clothing, and linens when wet or soiled.
• Assist residents in preparing for medical tests (lab work, x-ray, therapy, dental).
• Assist with lifting, turning, moving, positioning, and transporting residents.
• Perform restorative and rehabilitative procedures as instructed.
• Prepare residents for physical examinations.
• Check residents routinely to ensure personal care needs are met.
• Observe and report pressure areas, skin breakdowns, or injuries of unknown source.
• Observe disoriented and comatose residents; record and report data as instructed.
• Perform special treatments as instructed.
• Report residents leaving or missing from the facility immediately.
• Use only equipment you have been trained to use; report defective equipment.
• Report allegations of resident abuse or misappropriation of property.
• Honor resident’s refusal of treatment and report such requests to supervisors.

Requirements

• Minimum 10th grade education.
• Ability to read, analyze, and interpret professional journals, technical procedures, or regulations.
• Ability to write reports, business correspondence, and procedure manuals.
• Ability to present information and respond to questions from managers and employees.
• Ability to apply mathematical concepts (fractions, percentages, ratios) to practical situations.
• Ability to solve practical problems with limited standardization.
• Ability to interpret instructions in written, oral, diagram, or schedule form.
• Licensed Certified Nursing Assistant in accordance with state laws.
• CPR certification preferred.

Physical Demands

• Occasionally lift and/or move up to 25 pounds.
• Prolonged use of desktop or laptop computer.
• Regularly required to sit, stand, walk, talk, read, or hear.
• Frequent use of office equipment (copier, scanner, fax, telephone, calculator).
• May assist in evacuating residents during emergencies.

Work Environment

• Low to moderate noise level.
