Essential Duties And Responsibilities
- Performs basic nursing care procedures under the direction and supervision of a licensed nurse.
- Follows treatment plan designed to meet the appropriate age, needs and abilities of each resident/patient, including assisting with meals and nourishment, transferring, ambulation, bathing, toileting and dressing and grooming.
- Observes and communicates pertinent information about the resident/patient’s condition to the appropriate facility staff.
- Takes and records pulse, respirations, blood pressures and weight as needed.
- Documents care and observations on work sheets and EMR accurately and in a timely manner.
- Maintains cooperative relationships to enhance the efficient operation of the care team by providing support and assistance to co-workers.
- Adheres to infection control practices.
- Supports the psychosocial needs of the residents as per the plan of care and profile.
- Participates in educational and inservice meetings to update knowledge of techniques, guidelines and regulations.
- Must attend all mandatory programs and meets basic requirements for 12 hours of continuing education per year.
- Uses equipment in an appropriate and safe manner.
- Adheres to standards of the Diligent Minimal Lift Program.
Create and Implement Activity Plans
- Creates and implements activity plans for residents, including small groups, special events and entertainment.
- Motivates residents for participation in either planned groups or on a one-to-one basis.
- Engages residents in activities to improve their physical, social and emotional well-being.
- Enhances the quality of life of residents through engagement while promoting social interaction amongst residents.
- Observes and maintains safety of residents in falls program.
Qualifications
- Education: High School diploma or GED preferred.
- Experience: 6 months – 1 year experience as a CNA preferred.
- Certificates, Licenses, Registrations: State of Connecticut CNA Certification; or within 120 days of CNA Certification course completion.
