About the Role
- Providing direct patient care, including vital signs, meal assistance, and personal hygiene needs
- Assisting with resident transfers, ambulation, and routine care activities
- Maintaining accurate and timely documentation of patient care and progress
- Working closely with the healthcare team to ensure seamless coordination of care
- Demonstrating a passion for delivering exceptional patient care, while maintaining a positive and respectful attitude
Responsibilities
- Participate in afternoon and evening care routines, including vital signs, medication administration, and personal care activities
- Collaborate with the interdisciplinary team to develop and implement comprehensive care plans
- Assist with meal service, including setting up trays and serving meals
- Maintain a clean and organized environment, adhering to infection control and safety standards
- Communicate effectively with patients, families, and colleagues to ensure seamless care transitions
