Essential Duties And Responsibilities

  • Performs basic nursing care procedures under the direction and supervision of a licensed nurse.
  • Follows treatment plan that meets the appropriate age, needs and abilities of each resident/patient, including assisting with meals and nourishment, transferring, ambulation, bathing, toileting, and dressing and grooming.
  • Observes and communicates pertinent information about the resident/patient’s condition to the appropriate facility staff.
  • Takes and records pulse, respirations, blood pressures, and weight as needed.
  • Documents care and observations on work sheets and EMR accurately and in a timely manner.
  • Maintains cooperative relationships to enhance the efficient operation of the care team by providing support and assistance to co-workers.
  • Adheres to infection control practices.
  • Supports the psychosocial needs of the residents as per the plan of care and profile.
  • Participates in educational and inservice meetings to update knowledge of techniques, guidelines, and regulations.
  • Must attend all mandatory programs and meet the basic requirement for 12 hours of continuing education per year.
  • Uses equipment in an appropriate and safe manner.
  • Adheres to standards of the Diligent Minimal Lift Program.
  • Follows facility policies and procedures.
  • P Performs other duties as required.

Minimum Qualifications

  • Education: High School diploma or GED preferred.
  • Experience: 6 months – 1 year experience as a CNA preferred.
  • Certificates, Licenses, Registrations: State of Connecticut CNA Certification; or within 120 days of CNA Certification course completion.
