CNA | Certified Nursing Assistant | Center Aide | PACE Program | Part Time
About the role
Midland Care Connection is a not-for-profit, community-based organization dedicated to helping older adults and seriously ill individuals stay independent and supported in their homes. Established in 1978, it offers a full continuum of care, including hospice, palliative care, home health, grief and loss counseling, and its signature PACE (Program of All-Inclusive Care for the Elderly) program. The PACE program helps older adults remain safely in their homes rather than entering nursing facilities, providing complete medical care, rehabilitation, medications, transportation, social services, and supportive therapies coordinated by an interdisciplinary care team. A Certified Nursing Assistant working at the PACE Center supports participants with daily activities, monitors vital signs, assists with meals, accompanies participants during activities, and provides overall support to ensure comfort, safety, and independence in both the center and participants’ homes.
