CNA - Care Specialist
Responsibilities
- Facilitate personal care tasks like scheduling appointments, calling doctors’ offices, coordinating Medicare and other insurance details
- Implement ambulation methods, and assist patients in completing physical therapy and exercises as indicated in the care plan
- Administer basic medical tasks like taking vital signs, measuring glucose and ketone levels, replacing bandages and dressings, and ensuring the proper use of medical devices
- Perform light housekeeping tasks including but not limited to bed making, laundry, vacuuming, etc.
- Implement care plans as directed by the case manager, and keep updated and accurate care records
- Administer Medications
- Dispense medications as directed by the Registered Nurse (RN), maintaining an annual error rate of no more than one mistake
- Attend to any and all resident call lights within 10 minutes to ensure responsive and attentive care
- Perform light housekeeping tasks in resident rooms at least once weekly to promote a clean and comfortable living space
- Keep each room up to high standards of sanitization by changing bedpans and sheets, and performing regular room cleaning
- Service Charting
- Accurately document all services provided without missing tasks, ensuring compliance with company standards
- Support residents in Activities of Daily Living (ADLs) according to each individual’s Client Service Plan, with a zero-tolerance policy for missed tasks
- Give patient support with basic care, daily living activities, bathing, getting dressed, going to the restroom, eating regular meals, and taking proper medication dosages
- Encourage and initiate activities to enhance residents' mental and physical health and promote socialization

- CBRF Certifications or willingness to obtain them during employment
