Position Overview
Transitions At Home, a non-profit home health organization serving Wisconsin, is currently seeking a Part-Time CNA/Home Health Aide/Caregiver in the Sauk City area. The role involves providing hands-on personal care, companionship, and support to clients in their homes.
Responsibilities
- Provide personal care, bath, and hands-on care to clients
- Perform assigned functions of personal care under the direction, instruction, and supervision of the registered nurse (RN)
- Report changes in condition and needs to the RN Case Manager in accordance with the patient’s physician orders and plan of care
- Offer flexible hours and a supportive work environment
Qualifications
- Must meet Medicare Conditions of Participation for Home Health Aide training program and competency
- Current Wisconsin CNA License
- Sympathetic attitude towards the care of the sick and elderly
- Ability to carry out directions, read, and write
- Maturity and effective handling of job demands
