About the Role

Provide routine daily nursing care and services to assigned residents in accordance with each resident’s assessment and care plan, under the direction of supervisors.

Responsibilities

• Identify residents using wristbands or photo cards before administering treatments or serving meals.
• Use only authorized facility abbreviations when recording information.
• Report all changes in resident condition to the Nurse Supervisor/Charge Nurse promptly.
• Record entries on flow sheets, notes, and charts in an informative and descriptive manner.
• Report all accidents and incidents observed during the shift.
• Protect resident’s protected health information and report any suspected or known violations.
• Report unauthorized attempts to access the facility’s information system.
• Perform only nursing care procedures for which you have been trained.
• Prepare resident rooms for new admissions (e.g., bed made, name tags, admission kit available).
• Greet residents, escort them to their rooms, and introduce them to roommates and staff.
• Inventory and mark resident’s personal possessions; assist with packing/unpacking during transfers or discharges.
• Transport residents to new rooms or receiving areas; assist with loading/unloading residents to/from vehicles.
• Maintain a warm, personal, and calm environment for residents.
• Meet regularly with nursing personnel to identify and correct problem areas or improve services.
• Report all resident complaints and grievances.
• Participate in and receive nursing reports as instructed.
• Follow established policies concerning exposure to blood/body fluids.
• Make beds, measure and record vital signs (temperatures, pulses, respirations), and weigh residents as instructed.
• Check on residents unable to call for help frequently; answer resident calls promptly.
• Assist residents with meals (e.g., identifying food arrangements, recording food/fluid intake).
• Keep residents’ water pitchers clean, filled, and within reach; perform after-meal care.
• Perform all assigned tasks in accordance with established policies and procedures.
• Cooperate with inter-departmental personnel to maintain adequate nursing services.
• Notify the facility of late arrivals or absences.
• Report occupational exposures to blood, body fluids, infectious materials, or hazardous chemicals.
• Assist residents with daily functions (e.g., dental/mouth care, bathing, dressing, grooming).
• Keep residents dry by changing wet or soiled gowns, clothing, and linens.
• Assist residents in preparing for medical tests (e.g., lab work, x-rays, therapy).
• Lift, turn, move, position, and transport residents using proper techniques and equipment.
• Perform restorative and rehabilitative procedures as instructed.
• Assist in preparing residents for physical examinations.
• Monitor and report pressure areas, skin breakdowns, or injuries of unknown source.
• Observe and report conditions of disoriented or comatose residents.
• Perform special treatments as instructed; report missing residents immediately.
• Use only equipment you have been trained to operate; report defective equipment.
• Maintain confidentiality of all resident care information; report unauthorized disclosures.
• Report allegations of resident abuse or misappropriation of resident property.
• Honor resident’s refusal of treatment and report such requests to supervisors.

Requirements

• Minimum 10th grade education.
• Licensed Certified Nursing Assistant (CNA) in accordance with state laws.
• CPR certification preferred.

Skills

• Ability to read, analyze, and interpret professional journals, technical procedures, or governmental regulations.
• Ability to write reports, business correspondence, and procedure manuals.
• Ability to effectively present information and respond to questions from managers and employees.
• Ability to apply mathematical concepts (fractions, percentages, ratios, proportions) to practical situations.
• Ability to solve practical problems with limited standardization and interpret instructions in written, oral, diagram, or schedule form.

Pay

$18–$24 per hour.

Physical Demands

• Occasionally lift and/or move up to 25 pounds.
• Prolonged use of desktop or laptop computers.
• Regularly required to sit, stand, walk, talk, read, or hear.
• Frequent use of office equipment (copier, scanner, fax, telephone, calculator).
• May assist in evacuating residents during emergencies.
