About the role

Certified Nursing Assistant (CNA) providing home care services in patients' residences. The position involves travel within a designated area and requires adaptability to diverse client environments. The CNA is responsible for providing compassionate, patient-centered care while adhering to all safety and confidentiality standards.

Responsibilities

• Deliver direct patient care, including personal hygiene, transfers, repositioning, and ambulation assistance.
• Administer basic nursing care and prescribed exercises under supervision.
• Prepare meals considering client preferences and dietary needs.
• Offer companionship and engage clients in activities tailored to their interests.
• Perform light housekeeping and maintain accurate documentation of care activities.
• Monitor and report changes in clients' physical or psychosocial conditions.
• Utilize EVV app and timesheets for task documentation.
• Ensure confidentiality, uphold patient rights, and follow the care plan strictly.
• Respond to client needs, assist in promoting independence, and collaborate with team members.
• Maintain equipment such as blood pressure cuffs and stethoscopes, and adhere to safety protocols.

Qualifications & Skills

• Current DFS certification.
• High school diploma or GED preferred.
• Effective communication skills, ability to read, write, and follow directions.
• Physical ability to lift approximately 50 lbs, bend, stand, and perform necessary tasks.
• Compassionate attitude towards patient care.
