About the role
Under the direction of the licensed nurse, this individual will assist with the initial nursing intake on new admissions, gather demographic data, and ensure referral documents are present. This role involves scheduling appointments for clients with outside providers, completing internal paperwork such as trip logs and referral forms, and performing routine monitoring like vital signs and weights. The individual will also monitor self-administration of medications, maintain inventory of supplies, assemble medical charts, and conduct regular audits of medical records for compliance.
Responsibilities
- Conduct routine vital sign and weight assessments on clients and document according to protocol.
- Under direction, conduct initial intake interviews on new admissions, collecting demographic data, reported medical and mental health information, and presence of referring documents and/or medication.
- Communicate all information obtained to licensed nursing personnel.
- Provide supervision to clients who self-administer medications, including documentation in the medication logs.
- Maintain inventory of supplies, reordering as needed.
- Maintain medical/health-related forms, assembling new charts, and providing a supply of prepared forms.
- Audit medical records monthly.
- Assist in scheduling appointments with healthcare providers and complete associated paperwork, such as trip logs and referral forms.
- Triage client health complaints/requests and communicate with nursing personnel.
- Maintain CPR/First Aid certification and provide assistance when needed.
- Other related duties as assigned by the supervisor, Director of Health Services, or Program Director.
Schedule
- Required to work weekends at least once a month, including one off-shift per week to ensure program coverage.
- May be required to work shifts outside of assigned shifts during critical staffing shortages.
- Minimum holiday requirement includes 1 major summer holiday (Memorial Day, July 4th, or Labor Day) and 1 major winter holiday (Thanksgiving, Christmas, or New Year's Day; night shift: New Year's Eve). Other holidays will be rotated among all staff.
- This position is deemed essential and will be required to work during inclement weather.
Qualifications
- Active CMT (Certified Medical Technician) certification required.
- Minimum of two (2) years of supervised experience in a healthcare setting, preferably working as a Certified Medical Technician.
Skills
- Knowledge of healthcare practices such as monitoring vital signs and physical health indicators.
- Knowledge of safe infection control practices.
- Knowledge of correct record-keeping according to agency and regulatory guidelines.
- Ability to work with clients to gain confidence and cooperation.
- Ability to establish and maintain effective working relationships with clients, staff, outside healthcare providers, and the general public.
- Ability to communicate effectively with nursing and program staff to provide competent and appropriate healthcare to clients.
