Responsibilities

• Manage care for residents throughout the care continuum, including pre-admission, admission, and post-admission stages.
• Coordinate services to ensure that residents' needs are met in a timely and effective manner.
• Collaborate with healthcare providers, families, and interdisciplinary teams to develop and adjust care plans based on resident needs.
• Monitor the progress of residents and ensure appropriate interventions are implemented when necessary.
• Facilitate smooth transitions of care, ensuring all services and resources are in place for each resident.
• Advocate for residents and families, ensuring their needs are met, and provide education as needed.

Requirements

• Successfully graduated from an approved and certified LVN program.
• Current and unencumbered license to practice as an LVN in the state.
• Ability to make quick judgments and work independently.
• Knowledge of nursing practices, procedures, terminology, laws, regulations, and guidelines that pertain to long-term care.

Qualifications

• 2 years of experience as an LVN/LPN (nice to have).
• Active patient or bedside care experience within the past three years (nice to have).

Pay

Salary Range: $40,000 - $43,000 per year.
