Clinician NOC Crisis Stabilization Unit (ACSW/AMFT or LCSW/LMFT)

About the role

Amber House CSU is a 10-bed voluntary-only CSU whose purpose will be to assess individuals who are having a mental health crisis and are in need of assessment, stabilization, and brief treatment. The service is available to individuals for up to 24-hours. Amber House CRT has up to 14-beds for individuals in crisis who do not meet medical necessity criteria for hospitalization and would benefit from treatment and supportive programming.

The Clinical Care Coordinator is responsible for developing and providing high-quality therapeutic interventions and services in support of an individual’s wellness and recovery, intake assessment, leading group counseling, milieu management. Full Time opportunities: Overnight Shift 12 midnight to 8am, Tues to Saturday or Sunday to Thursday.

BACS clinical care coordinators get 40 hours per week of service hours toward licensure, 1 hour of individual supervision and 2 hours of group supervision per week.

Responsibilities

• Develop and deliver a full complement of services, including individual, family, group counseling, individual rehabilitation, and case management/brokerage for individuals within the Clinical Care Coordinator's caseload as assigned.
• Utilize Evidence-Based Practice for the designated population/program to assure high-quality results and intended impact.
• Continuously assess an individual’s risk status including risk-based symptoms, substance use, environment, complex physical conditions, etc, and intervene appropriately.
• Assess the continued service necessity and create a service plan that aims at an individual getting better/healthy and accomplishing service goals that are person-centered and belong to the client.
• Provide continuous outreach and progressive engagement and report disengagement to the team immediately.
• Work with the outreach team and determine when to disenroll individuals based on program protocols.
• Conduct domain-based assessment, screening referrals, service planning, community planning, and benefits coordination which supports wellness and recovery principles.
• Provide care planning and therapeutic case management using a whatever-it-takes approach.
• Document and maintain all data collection, reporting, and charting records in accordance with BACS and other relevant county regulatory policies/agencies.
• Produce the required number of regulatory units of service per month (65% of staff’s time must be provided face-to-face or other eligible community-based support).
• Participate in supervision, staff meetings, consultation, travel, crisis/emergency coverage, weekend coverage, as well as, agency-wide meetings and training, as required by the manager.
• Perform other duties as assigned.

Qualifications

• Master's Social Work (MSW), Master's Marriage Family Therapy (MFT) or Master's Practitioner in Clinical Counseling (MPCC) required.
• Associate ASW/ Licensed LCSW or Associate MFT/ Licensed LMFT with current BBS registration.
• 2 years of social service experience required.
• Bilingual in a threshold language preferred.
• Valid California driver’s license, personal vehicle, and proof of personal vehicle insurance coverage are required.

Skills

• Direct service experience working with older adults, forensic/re-entry, TAY or adults through previous employment, internships, and/or lived experience.

• Document services effectively and efficiently.
