Job Summary

Works in coordination with interdisciplinary teams to organize, prepare, and deliver evidence based clinical services across the continuum of care. Maintains ethical principles and professional standards of practice in adherence to the discipline’s professional code of ethics.

Job Responsibilities

  • Counsels and supports patients and families coping with illness, trauma, or end-of-life care.
  • Makes referrals to appropriate levels of care and available resources.
  • Provides education to patients and families regarding disease prevention, impact of illness, and adherence to treatment regimens.
  • Maintains timely patient-centered documentation reflecting pertinent information for assessment, interventions, and outcomes.
  • Participates in administrative and development activities including team meetings, clinical supervision of staff, and staff education.
  • Supervises social work field education for second-year students.
  • Supports the development of hospital and/or unit-based programming.
  • Contributes to improved patient experience.
  • Ensures cost-effective utilization of resources.

Qualifications

  • Master's Degree in Social Work, Counseling, Marriage and Family Therapy, or Related Human Service Field
  • Previous experience in a healthcare setting
  • LCSW, LPC, LMFT, LADC, PsyD, PhD
  • Valid Driver's License
  • Computer proficiency
  • Ability to assess problems, determine solutions, and make decisions within a timely manner
  • Excellent communication skills, with the ability to establish relationships within and outside the entity
  • Competencies including knowledge and behaviors to meet age-specific patient needs
  • Personal, patient/client, and unit/staff safety
  • Self-improvement through identification, communication, and seeking resources or instruction/validation
  • Cultural competence with an understanding of diverse client groups
