Responsibilities
- Provide home-based individual therapy, family therapy, and case management services to children, adolescents, and families.
- Develop and implement personalized treatment plans based on thorough clinical assessments.
- Conduct risk assessments, safety planning, and provide crisis intervention as needed.
- Maintain accurate and timely documentation in the Electronic Health Record (EHR), including progress notes, treatment plans, and discharge summaries.
- Ensure services are client-centered, evidence-based, and meet medical necessity criteria.
- Collaborate with a multidisciplinary team, including Therapeutic Mentors, Therapeutic Training and Support (TT&S) staff, and In-Home Therapists (IHT), to provide comprehensive care.
- Adhere to confidentiality standards, HIPAA regulations, and ethical guidelines in all client interactions.
- Participate in clinical supervision, team meetings, and professional development opportunities.
- Meet productivity expectations by maintaining a caseload, which may include evening hours.
Qualifications
- Master’s degree in Social Work, Psychology, or a related field that qualifies you for licensure in Massachusetts (e.g., LCSW, LICSW, or LMHC).
- Active CANS certification required.
- Experience providing home-based or community-based mental health services preferred.
- Proficiency in clinical assessments, treatment planning, and case management.
- Strong communication, organizational, and documentation skills.
- Knowledge of evidence-based therapeutic modalities and trauma-informed care.
- Able to maintain professional boundaries, confidentiality, and ethical standards.
