Clinician/Care Manager (Union) 26-49

Union position located at all sites, 40 hours per week.

About the role

This position provides intensive therapy and case management assistance/support to individuals and families in need of services provided by MI Services/SED Services. This may include case management, on-call, intakes, crisis management, and individual and/or group therapy. The role involves supporting other county agencies or traveling to individuals’ homes.

Responsibilities

• Provide individual, family, and group therapy; develop materials for and lead group therapy as needed.
• Manage an outpatient services caseload split between children and adults to meet agency needs.
• Link, monitor, coordinate, and advocate with community agencies to assist individuals in accessing basic needs such as housing, medical appointments, and employment.
• Participate in evidence-based practices (e.g., TF-CBT, ACT/IDDT/FPE).
• Complete intake assessments and make referrals for services and programs.
• Gather and review medical, psychological, and social history to determine individual needs and capabilities.
• Initiate and coordinate professional assessments; share and present assessment/treatment data as authorized.
• Maintain agency standards of compliance in documentation, billing, and other requirements for maximum reimbursement.
• Develop and periodically review person-centered plans of service, ensuring individual preferences are reflected.
• Make direct/indirect contacts in accordance with the person-centered plan to ensure implementation and monitoring.
• Serve as the primary advocate and liaison for assigned individuals, their families, guardians, or agencies.
• Provide consultation, referral, and in-services to staff, caretakers, families, and other agencies.
• Assist with placement planning and complete necessary forms and paperwork.
• Provide community outreach to individuals with a history of hospital recidivism to prevent crisis situations.
• Participate in after-hours on-call, crisis intervention during the workday, and other assigned duties.
• Carry out CCBHC clinical duties as assigned.

Children’s Outpatient Services

• Complete ADOS-2 assessments for the ABA benefit program as directed.
• Provide services based on the Children’s Mobile Crisis Team model to divert inpatient hospitalization for children under 21.

Children’s Home Based Services

• Provide services based on the Children’s Mobile Crisis Team model to divert inpatient hospitalization for children under 21.
• Provide family and individual therapy.
• Link and coordinate services (e.g., school, probation, DHHS).
• Provide family psychoeducation.
• Participate in BTC committee as needed for assigned individuals.

ACT (Therapist)

• Serve as team clinical lead for clinical consults.
• Provide family and individual therapy.
• Participate in BTC committee as needed for assigned individuals.

Residential Services

• Train staff working in homes.
• Monitor AFC home incident reports and medication compliance.
• Attend AFC home staff meetings.
• Serve as backup clinical consultant for behavioral issues.
• Develop and assess behavioral plans.
• Complete and coordinate residential treatment plans; visit homes at least monthly to monitor progress.
• Review residential progress notes for individuals in specialized placements.

Adult Outpatient Services

• Complete appropriate OBRA Assessments and/or provide OBRA coordination as directed.
• Create and implement behavior plans.
• Participate in BTC committee as needed for assigned individuals.

Intensive Outpatient Services

• Coordinate transition of services from ACT to intensive outpatient services.
• Provide higher-intensity services in office, home, and community to prevent inpatient hospitalization.
• Coordinate transition to traditional outpatient services when appropriate, ensuring least restrictive environment.
• Provide services in a multi-disciplinary approach with psychiatrists, care managers, peer support specialists, etc.

SUD/Integrated Health Services

• Facilitate IDDT evidence-based practice.
• Coordinate services with Medication Assisted Treatment programs within the county.
• Link individuals to community referrals, including SUD self-help groups and inpatient SUD treatment.
• Coordinate services with PCPs to assist in integrated treatment for comorbid health conditions and SUD diagnoses.
• Educate individuals on the 12-step program.
• Facilitate groups specific to individuals with co-occurring SUD/MH diagnoses.

Requirements

• Master’s Degree in a Human Services Field from an accredited school.
• Licensure as LMSW, LLMSW (preferred), TLLP, LLP, LP, LPC, or LLPC. LMSW preferred for Adult Outpatient Services.
• Qualify as QMHP, QMRP, or CMHP where appropriate for the population served.
• SUD license required for hires/transfers after 07.29.2025 (may comply by applying for MCBAP Plan within 60 days of hire, scheduling SUD licensure test, or becoming fully licensed).
• For work with children: 40 hours of Children’s training per calendar year and CAFAS rater certification.
• Minimum one year of experience with examination, evaluation, and treatment of SED children and families.
• For ASD Benefit Program assessments: ADOS-2 training or 1–2 years of relevant experience.

Pay

Hourly range: $31.84–$37.70. Annualized range: $66,219.38–$78,414.34.

Enhancements: $1.00 for full professional license; $0.50 for SUD license; $1.50 for both.
