Clinical Treatment Case Manager (Greenbrook)

We're looking for a compassionate, organized, and proactive Case Manager to serve as the primary point of contact for patients throughout their treatment journey.
Responsibilities
- Guide patients through their treatment journey, providing a seamless and supportive experience from consultation to completion
- Coordinate appointments, provider schedules, authorizations, specialty pharmacy activities, and treatment readiness requirements
- Monitor patient progress, address barriers to care, and collaborate with cross-functional teams to keep treatment plans on track
- Maintain accurate documentation, financial estimates, and patient records within CRM and EHR systems
- Respond to patient inquiries with empathy, professionalism, and urgency while delivering exceptional service
- Support high-quality patient outcomes while ensuring compliance with operational, coverage, and treatment requirements
Requirements
- High School Diploma or equivalent required combined with 3-5 years of experience in clinical case management, patient care coordination, direct patient care, or a related healthcare setting
- Experience using EHR systems and Microsoft Office applications (Outlook, Word, and Excel)
- Strong communication, organizational, and patient service skills with the ability to manage multiple priorities
Qualifications
- Associate's or Bachelor's degree in a healthcare-related field, with experience in patient care coordination, case management, or healthcare operations
- Experience with CRM systems (Salesforce preferred) and knowledge of insurance benefits, prior authorizations, healthcare scheduling, specialty pharmacy, or Buy & Bill processes
- Experience in behavioral health, specialty healthcare, TMS, Spravato®, or other specialty treatment programs preferred
