Clinical Supervisor (RN) - Kaplan Family Hospice Residence

Job Summary

The Clinical Supervisor at Hospice of Orange & Sullivan Counties is responsible for overseeing the day-to-day clinical operations of the Kaplan Family Hospice Residence, providing leadership, guidance, and support to clinical staff while ensuring the delivery of safe, high-quality, patient-centered hospice care.

Key Responsibilities

• Provide day-to-day clinical leadership and oversight of the Kaplan Family Hospice Residence, ensuring the delivery of safe, high-quality, patient-centered hospice care.
• Manage staffing, scheduling, and clinical coverage to support patient needs and operational excellence within a 24/7 care environment.
• Collaborate with physicians, nurse practitioners, and interdisciplinary team members to coordinate admissions, discharges, changes in level of care, and individualized care plans.
• Serve as a clinical resource and mentor to Residence staff, providing guidance, coaching, and support related to hospice and end-of-life care.
• Promote compliance with CMS, Joint Commission, NYS Department of Health, and organizational standards through clinical audits, quality monitoring, and survey readiness activities.
• Lead quality improvement, infection prevention, medication management, and patient safety initiatives to support exceptional clinical outcomes.
• Oversee staff orientation, competency validation, performance management, and professional development to foster a culture of continuous learning and accountability.
• Support recruitment, employee engagement, interdisciplinary communication, and organizational initiatives that enhance patient care and operational effectiveness.

Requirements

• Education/Credentials: Graduate of an accredited school of nursing, Bachelor's degree in Nursing (BSN), Valid unrestricted New York State Registered Nurse (RN) license, CPR/BLS certification, CHPN certification or must be obtained within 12 months following hire date.
• Experience: Five (5) years of progressive clinical nursing experience, Two (2) years of experience in hospice, palliative care, home care, oncology, long-term care, or a related healthcare setting preferred, Supervisory, leadership, charge nurse, or management experience preferred, Experience with staff scheduling, performance management, and clinical supervision, Experience with interdisciplinary care planning and coordination.
