Clinical Supervisor: Physical Therapy Home Health

Responsibilities

• Serves as the first point of contact for field clinicians to ensure support is easily accessible in a timely manner.
• Supervises clinical personnel and ensures appropriate care and services are delivered.
• Oversees clinicians providing patient care to patients in their place of residence.
• Handles incoming calls, follows up as necessary, and ensures appropriate actions are taken to meet internal and external customer needs.
• Coordinates training and educational programs for nurses within the field to improve clinical outcomes and competencies.
• Affords assistance in patient care escalations and supports the clinical team in discussing patients' progress and revising care plans as needed.
• Reviews and evaluates each case, assists clinicians in establishing therapeutic goals, setting priorities, and developing a plan of care.

Requirements

• Two years management or supervisory experience in a rehabilitation therapy department or program, preferably home care.
• Possesses a degree from a baccalaureate or masters program in applicable discipline approved by an accredited organization.
• Current CA Physical Therapist license.
• Valid CA Drivers License with an acceptable driving record.

Qualifications

• Knowledge of rehabilitation therapy principles and practices.
• Ability to communicate effectively with patients, families, and other healthcare professionals.
• Strong organizational and problem-solving skills.
• Experience with patient care documentation and record-keeping.

• Proficiency in Microsoft Office Suite.

Pay

Starting pay is commensurate with relevant experience above the minimum requirements. Clinical Supervisor: $143,000 - $163,000 per year plus a generous bonus, paid quarterly.

Schedule

This is a hybrid position combining field work and on-site responsibilities. The Clinical Supervisor - Therapy works 2 days in the Roseville or Natomas office (TBD) and 3 days in the field, visiting 12 patients weekly (including supervisory visits).
