About the role
At 21st Century Home Health Services (21HHS), we are the largest home health agency in San Francisco and the fastest-growing in the Bay Area. We care for more than 4,000 patients across multiple counties and are actively expanding into Marin and Sonoma counties. Our clinicians prioritize both patient care and one another, maintaining low hospital readmission rates and high employee satisfaction.
Responsibilities
- Ensure patient care is coordinated and managed appropriately
- Serve as the first point of contact for field clinicians to ensure support is easily accessible in a timely manner
- Supervise clinical personnel and oversee clinicians providing patient care to patients in their place of residence
- Oversee clinicians in the field to ensure appropriate care and services are delivered
- Assist in patient care escalations and coordinate Case Conferences with the clinical team
- Review and evaluate each case, establish therapeutic goals, and develop a plan of care
Requirements
- Two years management or supervisory experience in a rehabilitation therapy department or program, preferably home care
- A degree from a baccalaureate or masters program in applicable discipline approved by an accredited organization
- A current CA Physical Therapist license
- A valid CA Drivers License with an acceptable driving record
Qualifications
- Current Basic Life Support (BLS)

Pay
Starting pay is commensurate with relevant experience above the minimum requirements. Clinical Supervisor: $143,000 - $163,000 plus a generous bonus, paid quarterly.
Schedule
This is a hybrid role with 2 days in the Burlingame office (TBD) and 3 days in the field, visiting 12 patients weekly (supervisory visits of field clinicians).
