Responsibilities

• Serve as the first point of contact for field clinicians to ensure support is easily accessible in a timely fashion.
• Ensure that patient care is coordinated and managed appropriately.
• Oversee the supervision of clinical personnel and the delivery of care and services.
• Handle incoming calls timely and follow up as necessary to meet internal and external customer needs.
• Coordinate training and educational programs for nurses within the field to improve clinical outcomes and competencies.
• Assess staff learning needs and formulate specific plans and goals for the next evaluation period with employees.
• Perform administrative call duties and serve as a role model and resource for staff.
• Function as the first point of contact for field clinicians to assist in patient care escalations.
• Coordinate at least bi‑weekly Case Conferences with the clinical team to discuss patients' progress and revise care plans as needed.
• Review and evaluate each case by reviewing services provided by clinicians, conference notes, and records; discuss and verify impressions; instruct and guide clinicians to promote more effective performance and delivery of quality home care services.
• Be available during operating hours to assist clinicians as appropriate.
• Review patient’s clinical diagnosis, medications, procedures, and clinical course.
• Assist clinicians in establishing immediate and long‑term therapeutic goals, setting priorities, and developing a plan of care.
• Conduct supervisory visits: 12 patient visits each week in the field.

Requirements

• Two years of management or supervisory experience in a rehabilitation therapy department or program, preferably home care.
• Possess a degree from a baccalaureate or masters program in an applicable discipline approved by an accredited organization.
• Hold a current California Physical Therapist license.
• Have a current Basic Life Support (BLS) certification.
• Possess a valid California Driver’s License with an acceptable driving record.
• All licenses and certifications must be current at time of hire and sustained throughout employment.

Pay

Wage range for Clinical Supervisor: $143,000.00 – $163,000.00 plus a generous bonus, paid quarterly. Starting pay is commensurate with relevant experience above the minimum requirements.

Schedule

Hybrid schedule: 2 days in the Walnut Creek office and 3 days in the field, with 12 patient visits each week (supervisory visits of field clinicians).
