Clinical Supervisor Care Management MHB

Responsibilities

• Supervise the day-to-day operations of the Care Management Department
• Achieve site-specific and system-wide discharge planning performance metrics
• Provide clinical oversight to the Care Management staff
• Serve as a subject matter expert and coach in discharge planning, care coordination, patient/family concerns, insurance issues, and discharge to other levels of care

Requirements

• Bachelor's degree in Nursing required
• Registered RN in New York State
• Specialty certification preferred (e.g., CCM)
• Three (3) years of care management/medical management experience
• Demonstrated success and accountability for clinical, quality, and financial results in an acute care or managed care environment
• One (1) to two (2) years of management/supervisory experience preferred

Knowledge, Skill and Ability

• Demonstrated ability to think autonomously, make strategic decisions, and balance multiple priorities
• Excellent critical thinking, complex problem-solving, and analytical/statistical skills
• Extensive knowledge of medical management, utilization, discharge planning principles/practice
• Extensive knowledge and fluency of NYS DOH, CMS, JCAHO standards, third-party regulations, and compliance standards
• Recognized and viewed as a subject matter expert on care management, care coordination, disease management, discharge planning, utilization review for respective site

Working Conditions

• Routine travel between acute care facilities may be required
• Flexibility in scheduling and on-site assignments
• Normal heat, light space, and safe working environment
