Responsibilities

• Coordinate participant appointments and services, including care provided outside of the PACE center.
• Work with external vendors, providers, and community partners to schedule and coordinate participant services.
• Communicate with participants, families, and the care team to keep appointments and services on track.
• Help ensure participants have access to the care, transportation, and support services they need.

Requirements

• High School diploma equivalency OR 1 year of applicable cumulative job specific experience required.
• Note: Required professional licensure/certification can be used in lieu of education or experience, if applicable.

$21.31 - 28.84 per hour
