The Clinical Pharmacist provides specialized pharmaceutical services to improve drug usage and therapeutic outcomes for neonatal, pediatric, adolescent, adult, and geriatric patients. This role actively participates in multidisciplinary patient-care rounds, medication order evaluation, drug distribution, technician supervision, and clinical services including medication histories, pharmacokinetic dosing, drug information requests, and patient counseling.

Responsibilities

• Delivers comprehensive pharmaceutical care to patients in collaboration with clinicians and healthcare providers through multidisciplinary rounds.
• Reviews medication orders for accuracy, appropriateness, and efficacy.
• Documents all pharmacy interventions in the patient electronic medical record and integrated clinical decision support system.
• Monitors drug therapy for appropriate use, dose, dosage form, regimen, route, therapeutic duplication, and drug interactions.
• Provides pharmacokinetic consultations and dosing recommendations.
• Prevents, detects, monitors, documents, and reports adverse drug reactions and medication errors.
• Participates in drug therapy management of medical emergencies (e.g., code blue, code stroke).
• Performs admission history and discharge medication review/counseling to facilitate patient transitions.
• Provides operational support to Clinical Staff Pharmacists as needed.
• Responds to drug information requests and optimizes pharmacotherapy regimens using evidence-based literature.
• Promotes rational, cost-effective drug therapy and participates in formulary management.
• Assists in medication procurement, including verification of home medication supply when applicable.
• Facilitates clinical and operational initiatives and documents consultations and cost avoidance.
• Prepares formulary reviews and drug use evaluations for the Pharmacy and Therapeutics Committee.
• Assists pharmacy leadership in managing alternative therapies during medication shortages.
• Facilitates and participates in scholarly and professional development activities.
• Provides guidance and supervision to certified pharmacy technicians.
• Precepts pharmacy students (IPPE/APPE rotations) and PGY1/PGY2 residents as qualified by ASHP standards.
• Participates in staff development, departmental research, and local/regional/national organizations.
• Collaborates with pharmacy leadership on education, training, and competency programs.
• Executes hospital and pharmacy policies and procedures, serving as a role model and educational resource.
• Assists with recruitment, orientation, and quality assurance activities (e.g., medication history audits).
• Investigates and resolves pharmacy management software-related issues.
• Covers assigned shifts as determined by departmental need.
• Facilitates investigational drug studies with support from the Investigational Drug Service Manager.
• Demonstrates advanced communication skills and forms collaborative relationships with healthcare teams.
• Participates in pharmacy department or hospital committees and maintains compliance with policies.

Requirements

• Graduated from an ACPE-accredited College of Pharmacy (required).
• Post-Graduate Year 1 (PGY-1) Pharmacy Residency or three (3) years of relevant work experience (required).
• Post-Graduate Year 2 (PGY-2) Pharmacy Residency (preferred).
• Licensed or license-eligible as a pharmacist in the Commonwealth of Kentucky (required).
• Current board certification by the Board of Pharmacy Specialties® or within 2 years of hire (required).
• Current certification in Basic Life Support (BLS) or must obtain within 90 days of hire (required).
• Current certification in Advanced Cardiac Life Support (ACLS) or must obtain within 90 days of hire (required).
