About the role
This position is located at California Health Care Facility in Stockton, San Joaquin County. The Clinical Social Worker (Health/CF) Safety conducts assessments, provides assessments, group therapy, and collateral conferences, and documents and evaluates patients' progress in treatment.
Responsibilities
- Conducts assessments and summarizes case information to use in diagnosis, treatment (level of care), and dispositional release
- Diagnoses and/or collaborates in the formulation of a diagnosis
- Develops, monitors, and modifies treatment plans in collaboration with the interdisciplinary treatment team
- Provides assessments, group therapy, and collateral conferences
- Documents and evaluates patients’ progress in treatment
- Congnrols placement with community resources and the Classification and Parole Representative

- Pay Differential: Employees in this class, employed within the California Department of Corrections and Rehabilitation, and who are physically located at and assigned to the Psychiatric Inpatient Program, shall receive a pay differential equivalent to 15% of their monthly base pay.
- Tuberculosis (TB) testing is required for candidates new to CCHCS prior to appointment followed by department annual TB testing/evaluation thereafter.

$8,678.00 - $10,569.00 per Month
