About the role

The Social Worker plays a crucial role in assessing and addressing the social, emotional, and environmental needs of patients and their families. They collaborate with multidisciplinary teams to develop and implement comprehensive treatment plans.

Responsibilities

  • Perform psychosocial assessments and develop treatment plans
  • Evaluate patients' medical conditions and assess client needs as part of the interdisciplinary team
  • Maintain and document client progress according to measurable goals
  • Collaborate with other professionals to manage patient care
  • Advocate for clients while maintaining professional boundaries
  • Provide evidence-based interventions and skill-based groups
  • Facilitate communication and coordination among the interdisciplinary team

Requirements

  • Master's Degree in Mental Health & Behavioral Medicine or Social Work
  • Licensed Independent Clinical Social Worker (LICSW), Licensed Mental Health Counselor (LMHC), or Licensed Clinical Social Worker (LCSW)
  • 2+ years of clinical experience in a medical setting
  • Class D Passenger Vehicle Driver's License

Pay

$32.47 - $47.23/Hourly

Schedule and Working Model

Per Diem. Available to work a minimum of 16 hours per week, preferably up to 40 hours per week. Hours fall between 8:30am-5pm Monday through Friday. Travel within the Boston, MA area required. Days available include M- F 8:30-5. No on-call, no weekends. Remote workdays require a stable, secure, quiet, compliant work area. This is a hybrid role where phone calls, follow-up calls, meetings, case discussions, and huddles can be conducted from home. The schedule seeing patients in the community varies from day to day and week to week. Social Worker must be flexible and able to see patients in the community as needed on all the days they commit to work.
