Clinical Social Worker - Outpatient Clinic (Neurology)

About the role

The Clinical Social Worker (Outpatient Neurology) is responsible for facilitating the adjustment of the patient and family to illness and helping them function optimally. Services include psychotherapy, psychosocial support, and coordination of community resources.

Responsibilities

• Manage complex assessments and establish goals for patients and families.
• Periodically reassess patient/family situations and modify plans as needed.
• Provide services addressing social needs such as domestic violence, psychiatric emergencies, child abuse, family disputes, end-of-life, and bereavement.
• Collaborate with interdisciplinary teams to assess and treat mental and behavioral health needs.
• Act as a liaison between hospitals and community agencies, providing aid to families and children.
• Coordinate and provide therapy sessions for patients needing counseling/therapy.
• Offer counseling and therapy to patients and families, addressing emotional, personal, psychological, and socioeconomic factors.
• Document interventions in the Electronic Medical Record following departmental guidelines.
• Ensure compliance with Medicaid, CMS, and other regulatory/compliance agency requirements for documentation and billing.
• Interface with insurance providers, case managers, and others involved in authorization of patient services.
• Establish and maintain effective working relationships across disciplines.
• Provide families with specific community resource information.

Requirements

• Masters Degree in Social Work from a CSWE-accredited program.
• Minimum three (3) years of experience required, including graduate school internship.
• An active LCSW license is required.
