Schedule
Monday - Friday 8:30 AM - 5:00 PM with alternating weekends of 8:00 AM - 6:30 PM
Demonstrates Knowledge
- Knowledge of techniques, procedures, use of equipment, and Hospital & departmental policies necessary for job performance.
Job Duties
- Uses screening mechanisms to identify patients at risk for lengthy inpatient stays, complex clinical issues, high resource use, or repeat admissions due to chronic diseases.
- Intervenes within 24 hours of patient admissions.
- Documents according to established guidelines.
- Assesses patient & family psychosocial needs, including coping mechanisms, ability to comply with plan of care, and desired outcomes.
- Makes referrals as appropriate.
- Provides patient & family counseling as appropriate.
- Collaborates with physicians and other health care team members to achieve desired outcomes per physicians’ plans of care.
- Collaborates as needed with the Utilization Management Committee.
- Identifies potential or current patient situations requiring referral to other members of the health care team such as financial counseling or risk management.
- Affirms discharge plan implementation and follow-up.
- Serves as a resource for patients, family members, and the community; provides information and referrals as needed.
- Facilitates discharge planning process and identifies need for referral to external sources for post-hospitalization needs.
- Maintains accurate & current knowledge of available community resources including home-health agencies, skilled-nursing facilities, rehab facilities, and other available resources.
- Explains options to patients & families.
- Maintains accurate & current knowledge of payor practices and regulations that may impact patient outcomes and/or progress.
- Initiates & maintains all acute-care abstracts for discharge planning/social services.
- Initiates & maintains MedQM data on all patients.
Qualifications
- Must be licensed by the Maryland Board of Social Work Examiners at the LMSW level, but an LCSW-C is preferred.
- Two or more years of licensed clinical social work experience in an inpatient hospital healthcare setting; MICU experience preferred.
- Aidin Discharge software experience is strongly preferred.
