About the Role

Under general supervision, and in addition to performing the full range of operational level duties, responsible for administering a licensed social service program designed to support engagement in care, treatment adherence, and optimal health outcomes for individuals living with HIV. Provides licensed social work services consisting of continuing care services and clinical social work services, including crisis intervention, mandated reporting, information and referral, coordination of health and mental health services with community providers. The social worker is responsible for education and advocacy of patients, families and/or significant others. Participates in related department functions, such as community and program education. Performs all other duties as assigned.

Minimum Qualifications

  • Master's degree from an accredited school of social work
  • Knowledge and abilities essential to the successful performance of the duties assigned to the position
  • Ability to work collaboratively as a team
  • Excellent interpersonal skills to work with ethnically, culturally and socioeconomically diverse populations
  • Must have proven independent thinking, decision making, problem solving and prioritizing skills
  • Excellent skills to communicate and influence effectively with all levels of staff, physicians, patients and external constituents, both verbally and in writing
  • Excellent ability to multi-task with demanding schedules
