About the Role

Join our team at Missouri Baptist Medical Center to make a direct impact on patient care and operations. This is a full-time position with a schedule of 40 hours per week, Monday through Friday, with limited holidays and weekends. Day shift hours are 8:00 AM to 4:30 PM.

Missouri Baptist Medical Center, an acute care hospital in St. Louis County, offers a full continuum of medical and surgical services, including heart care, cancer, women and infants, breast health, gastrointestinal, orthopedic, stroke, therapy, wound, and pain management services. The hospital has a 24-hour adult emergency department and cares for pediatric patients at a separate emergency department in collaboration with St. Louis Children’s Hospital.

Missouri Baptist is the first and only hospital in St. Louis County to be named a Magnet® hospital by the American Nurses Credentialing Center (ANCC), a prestigious designation achieved by less than nine percent of hospitals nationwide. Additionally, U.S. News & World Report ranked the hospital #2 in the St. Louis metro area.

The Care Coordination Department includes RN Case Managers, Social Workers, Compliant Documentation Specialists, and skilled administrative staff. Case Management in the hospital setting is a collaborative practice model involving patients, nurses, social workers, physicians, other practitioners, caregivers, and the community. The process encompasses communication and facilitates care along the continuum through effective resource coordination. The goals include achieving optimal health, access to care, and appropriate utilization of resources, balanced with the patient’s right to self-determination.

Responsibilities

  • Conducts screening and assessment activities to meet the needs of the discharged patient.
  • Applies assessment findings to prepare a patient-centered plan of care.
  • Applies and complies with all regulations and utilizes resources available to patients to manage their continuum of care.
  • Provides clinical social work services to address the psychosocial needs of patients and families, including individual and group counseling, linkage to community resources, expertise in cultural diversity, and advocacy.
  • Collaborates with the health care team and ensures timely, cost-effective discharge and transition planning.

Requirements

  • Master’s Degree in Social Work.
  • Licensed Clinical Social Worker (LCSW) in Missouri.
  • No prior experience required, though 2-5 years is preferred.
