Salary: $49.17/hr ($102,297.29 per year at 40 hrs/week) for licensed Clinical Social Workers; $38.93/hr ($80,973.40 per year at 40 hrs/week) for Associate Social Workers. Additional $1.00 per hour for employees certified bilingual in Spanish, Chinese, or Tagalog.

Hours: 32-40 hrs/week, with up to 1 remote day per week. Primary location: Lyon-Martin Community Health Services, 1735 Mission Street, San Francisco, CA 94103. FSLA Status: Non-Exempt.

The mission of Lyon-Martin Community Health Services is to provide high quality, compassionate, and trauma-informed medical, gynecological, and mental health care services to trans, non-binary, gender non-conforming, and intersex communities and cis-gender women. About the Role

The Clinical Social Worker (CSW) provides comprehensive case management services to patients of Lyon-Martin, working closely with a multidisciplinary team to assess patient needs, coordinate care, and connect patients to medical, mental health, and social services. The CSW delivers clinically appropriate case management and crisis intervention, including substance use disorder counseling, crisis de-escalation, safety planning, and referrals.

Responsibilities

  • Conduct comprehensive intake assessments, including medical, psychological, and social factors, to identify patient needs, goals, and social determinants of health (e.g., housing, employment, food access).
  • Develop individualized care plans addressing physical, mental, and social well-being in collaboration with patients, their families, and care teams.
  • Serve as a liaison between patients, healthcare providers, and external organizations to coordinate care, ensure shared consent, and empower informed decision-making.
  • Assist patients with scheduling appointments, arranging transportation (Paratransit, Tokens, Lyft), interpretation access, and navigating the healthcare system, including accompanying clients to appointments as needed.
  • Provide and coordinate referrals for wraparound services, such as housing (Coordinated Entry, Our Trans Home, Open House), financial assistance (CAAP, GA, guaranteed income programs, SDI, SSDI), food resources (SNAP, Project Open Hand, Meals on Wheels, Lyon-Martin’s Community Fridge), In-Home Support Services, and higher-level mental health services.
  • Provide immediate triage and intervention for patients presenting with suicidal ideation or mental health crises, including risk assessments, safety planning, and follow-up care.
  • Develop preventative strategies and conduct regular check-ins or warm handoffs to accessible mental health services.
  • Lead restorative justice and accountability conversations with clients who breach Lyon-Martin’s community agreements, focusing on understanding motivations, planning for future challenges, and agreeing on commitments to appropriate behavior.
  • Maintain accurate, timely documentation in Epic EHR, including visit notes and in-basket messages (within 72 hours, same day if urgent).
  • Monitor patient progress and update care plans to reflect changes in condition or situation.
  • Meet productivity benchmarks, maintaining a caseload of at least 3 paneled patients per half-day.
  • Adhere to FQHC, federal, state, and city regulations, including HIPAA, to ensure patient privacy and confidentiality.
  • Participate in weekly Case Management Team Meetings, Collaborative Care Meetings, All Staff Meetings, and applicable subcommittee meetings to improve patient experience and health outcomes.
  • Engage with clients who have high no-show rates or frequent emergency care utilization to support access to care.
  • Facilitate group therapy to foster safety, consistency, and community connection as needed.
  • Engage in professional development to stay informed on best practices in case management, crisis intervention, restorative justice, and patient care.

Requirements

  • Graduation from an accredited school of social work.
  • Licensed in California as an LCSW or ASW.
  • Extensive knowledge of Bay Area public benefit resources and networks.
  • Excellent de-escalation and non-violent communication skills.
  • Experience safety planning for individuals in crisis, particularly those with suicidal ideation.
  • Experience mediating using restorative justice principles and facilitating accountability conversations.
  • Strong interpersonal skills, with an emphasis on empathy and patient-centered care.
  • Culturally sensitive/humble and able to work with diverse populations.
  • Strong proficiency with EHR systems, Microsoft Office (Word, Outlook), internet applications, Zoom, and phone use.
  • Experience working with lesbian, bisexual, transgender, gender non-conforming, non-binary, and intersex communities, as well as populations with varying disabilities, racial, ethnic, and cultural backgrounds, economic statuses, people who do sex work, those experiencing homelessness, people with severe mental illness, people who use drugs, those with a history of incarceration, and people of varying ages.
  • Commitment to accessible gender-affirming health care, abortion, and primary care for marginalized populations.

Preferred Qualifications

  • LCSW licensure.
  • Bilingual Spanish/English fluency; other bilingual or multilingual fluency.
  • Experience using Epic EHR.
  • At least 2 years of experience providing case management in a community clinic or hospital setting.
