Clinical Social Worker - LCSW - Outpatient Dialysis

About the role

We are seeking a compassionate and dedicated Outpatient Dialysis Social Worker to join our multidisciplinary team. In this vital role, you will provide psychosocial support including counseling and case management, education, and advocacy for patients with chronic kidney disease receiving dialysis treatment on an outpatient basis or in the home setting. You will collaborate with medical professionals to ensure holistic, patient-centered care and help patients and their families navigate the complexities of chronic illness management.

Responsibilities

• Provide psychosocial support including counseling and case management, education, and advocacy for patients with chronic kidney disease receiving dialysis treatment on an outpatient basis or in the home setting
• Collaborate with medical professionals to ensure holistic, patient-centered care
• Help patients and their families navigate the complexities of chronic illness management
• Complete assessments and surveys per Mayo Clinic Policy/Procedure and CMS requirements
• Participate in dialysis clinic quality improvement processes

Qualifications

• Master of Social Work (MSW) or Master of Science in Social Administration (MSSA) degree
• Florida LCSW license to practice independently at the clinical level as required by applicable state requirements
• Advanced knowledge of mental illness, personality development, and the types of treatment, and skills in conducting individual, group, and family therapy
• Knowledge of adjustment to medical diagnosis and chronic disease
• Strong communication skills to interact with patients, patients' family members, consultants, residents, hospital personnel, community agencies, and other resources
• Ability to communicate effectively in both verbal and written form
• Psychosocial diagnostic skills

Pay

$71,510.40 - $107,390.40 / year; Education, experience and tenure may be considered along with internal equity when job offers are extended.

Schedule

Full Time, 80 hours per pay period. Monday - Friday 8:00 a.m. - 4:30 p.m. Flexibility for business need is required. No scheduled weekends but could be requested on business need.
