Clinical Social Worker - CSWA or LCSW

About the role

We are seeking Clinical Social Workers who are committed to holistic, patient-centered care and serving vulnerable populations. Our team at Legacy Mount Hood Medical Center provides a range of services including comprehensive evaluations, crisis intervention, and coordination of services.

Responsibilities

• Provides social work services in various adult and pediatric settings across the health care continuum.
• Works independently and collaboratively with interdisciplinary teams to provide comprehensive care.
• Conducts comprehensive evaluations of psychosocial and mental/behavioral health status, legal and safety considerations, substance use or addiction, chronic illness, or end of life issues.
• Utilizes a strengths-based approach to enhance the capacity of individuals, families, and groups to address their own needs.
• Coordinates with internal and community stakeholders to ensure seamless care delivery.

Qualifications

• Clinical Social Worker I (CSW A): Master of Social Work degree required; 1 year of relevant experience preferred.
• Licensed Clinical Social Worker II (LCSW): Master of Social Work degree required; 2 years of relevant experience preferred; 3 years preferred.

Skills

• Broad knowledge of social work principles and ability to integrate knowledge of normal development, illness, and treatment procedures.
• Strong organizational, communication, and collaboration skills.
• Knowledge of legal/regulatory issues and ability to advocate for patient/family access to resources.
• Ability to navigate electronic medical records and utilize basic computer programs.
• Understanding of social determinants of health and principles of diversity/equity/inclusion.

Benefits

• Pay range: USD $40.06 - USD $60.14 /Hr.
• American Heart Association (AHA) issued BLS Provider certification required.
