Clinical Social Worker - CSWA or LCSW

Responsibilities
- Evaluate the psychosocial and mental health needs of patients to support transition planning, crisis intervention, and coordination of both internal and external services.
- Assess and implement intervention strategies related to behavioral health, addiction, end-of-life care, and informed decision-making regarding therapeutic options.
- Serve as advocates for patients, families, caregivers, surrogates, and healthcare teams, ensuring compassionate, comprehensive care throughout the patient journey.
Qualifications
- Clinical Social Worker I (CSW A): Master of Social Work degree from an accredited School of Social Work required. Prefer 1 year of relevant experience in care management, mental health and/or addictions services, medical social work, or social service organization.
- Licensed Clinical Social Worker (LCSW): Master of Social Work degree from an accredited School of Social Work required. 2 years relevant experience in care management, mental health and/or addictions services, medical social work, or social service organization. Preferred: 3 years' relevant experience.
Skills
- Broad knowledge of the principles of social work and the ability to apply them with individuals, families and/or groups integrating knowledge of normal development and the aging process, changes related to illness, and the needs of patients with acute, chronic, and terminal illness.
- Knowledge of medical, mental health and substance use diagnoses, disabilities, and treatment procedures.
- Strong organizational, communication and collaboration skills, including an ability to effectively triage patient and family needs, function as member of an interdisciplinary team, identification and collaboration with community stakeholders or resources and written documentation.
- Knowledge about social determinants of health, principles of diversity/equity/inclusion and culturally responsive practice with populations who have experienced historical marginalization.
- Exhibits knowledge of legal/regulatory issues and an ability to be a resource to all team members regarding such issues.
- Knowledge of current health care environment.

- CSW A: USD $40.06 - USD $57.28 /Hr.
- LCSW: USD $42.06 - USD $60.14 /Hr.
