About the role

The Critical Care Nurse Navigator supports the critical care service line by coordinating with physicians, pharmacists, ancillary departments, and case management departments. They serve as a liaison and adjunct support for critical patients, focusing on time-critical diagnoses such as sepsis. This role involves corresponding and collaborating with physicians, nursing, and other healthcare providers across the continuum, responding to patient care areas including the Emergency Department, Medical, Surgical, or Cardiac floors, and the Intensive Care Unit.

Responsibilities

  • Triage and coordinate patients in the Emergency Department and on Medical, Surgical, or Cardiac floors to provide critical care expertise and collaboration with intensivists.
  • Ensure patients receive timely, safe, effective, and high-quality care.
  • Coordinate processes and enhance collaboration and quality clinical outcomes as evidenced by specified metrics, optimizing performance and adoption of evidence-based practice.
  • Provide support, mentorship, and guidance to patients and families, as well as members of the healthcare team.
  • Display a positive presence and adapt quickly in a changing environment.

Requirements

  • Graduated from a school of nursing (Associate's Degree, Diploma, or BSN).
  • Licensure: Possesses a current RN license within the hiring state and/or compact licensure in which nursing duties are performed and meets all state board of nursing requirements.
  • Basic Life Support certification through the American Heart Association or successful completion of course within 30 days of hire.

Qualifications

  • Confident in high-acuity situations and strong clinical decision-making.
  • Previous critical care experience preferred.
  • Ability to promote an environment conducive to learning for patients and other healthcare providers.

Skills

Effective communication skills, leadership qualities, and the ability to work collaboratively with multidisciplinary teams.
