Job Responsibilities
- Affirms and organizes patient care considering patient age, cultural, and psychosocial factors.
- Participates in the creation, review, and evaluation of multidisciplinary treatment plans according to policies and procedures.
- Develops a therapeutic relationship with assigned patients to integrate psychosocial treatment with standard physical nursing care and guidance.
- Maintains and reviews patients' records, including posting test and examination results.
- Facilitates patient discharge through timely and appropriate referrals and utilization of patient education resources, tailored to patient age and cultural/psychosocial factors.
- Collaborates with other disciplines as needed.
- Supports unit/hospital goals through participation in Performance Improvement (PI) and educational activities, as evidenced by attending in-services, meeting unit education standards, and completing variance reports.
- Performs other duties as assigned.
Required Skills & Experience
- Two (2) years of related experience.
- Working knowledge of general disease state management processes, bio-psychosocial model, and clinical pathways.
- Effective interpersonal, written, and oral communication skills.
- Ability to organize and prioritize work in an effective and efficient manner.
- Ability to be detail-oriented in the examination of clinical and numerical data.
Required Education & Certification
- Has completed the basic professional curricula of a school of nursing as approved and verified by a state board of nursing and holds or is entitled to hold a diploma or degree therefrom.
- Valid multi-state or State of Oklahoma Registered Nurse License.
