Key Responsibilities
- Lead and coordinate resident assessments (MDS), ensuring compliance with federal and state regulations.
- Champion quality care by developing individualized care plans and working closely with interdisciplinary teams.
- Oversee Medicare and managed care referrals and ensure accurate documentation and care coordination.
- Serve as a mentor, coach, and leader for clinical staff, promoting excellence in care delivery.
- Act as Chairperson of the Interdisciplinary Care Plan Team and attend care conferences with residents and families.
- Monitor clinical workflows, staffing levels, and resource utilization to meet resident needs efficiently and effectively.
Requirements
- Registered Nurse (RN) with a current, active license in this state.
- AANAC certification (or willingness to obtain within 6 months).
- Minimum 2 years of MDS experience preferred.
- CPR certified and skilled in clinical documentation and assessment tools.
- Strong leadership, communication, and collaboration skills.
Preferred Qualifications
- Bachelor of Science in Nursing (BSN).
- Knowledge of Care Tracker, PPS Medicare criteria, and long-term care regulations.
