About the role

ElderServe Health is a mission-driven, nonprofit managed care organization serving the New York metropolitan area. ElderServe provides long-term services and supports to more than 20,000 older adults and adults with disabilities across New York City, as well as Westchester, Nassau, and Suffolk Counties. ElderServe primarily serves a frail and elderly membership who is predominantly dual-eligible and enrolled in Medicaid managed long-term care, Medicare-only and integrated Medicaid-Medicare plans. The CareSource and ElderServe affiliation addresses these critical long-term care needs to help members be as healthy and independent as possible.

Responsibilities

  • Supports the implementation and day-to-day execution of rehabilitation program activities and workflows.
  • Aids in operational planning, coordination, and monitoring of program performance.
  • Applies established policies, procedures, and clinical guidelines in daily operations.
  • Identifies operational issues and escalates appropriately.
  • Collaborates with the Rehabilitation Director and VP to implement program initiatives, monitor performance, and ensure consistent application of rehabilitation practices.
  • Coordinates rehabilitation services to ensure timely, appropriate, and effective care delivery.
  • Sustains interdisciplinary care team discussions and addresses service-related issues.
  • Supports evaluation of member needs, including coordination for long-term care settings.
  • Supports utilization review activities to ensure services are appropriate and aligned with program standards.
  • Maintains documentation for completeness and alignment with regulatory and reimbursement requirements.
  • Identifies documentation or utilization issues and partners with leadership for resolution.
  • Supports quality assurance and performance improvement activities.
  • Ensures adherence to established regulatory, accreditation, and organizational requirements.
  • Assists with audit preparation and data gathering for regulatory reviews.
  • Collaborates with contracted providers and vendors to support service delivery and resolve issues.
  • Partners with internal teams (e.g., DME, care management) to address member care needs.
  • Communicates effectively with providers, members, and internal stakeholders.
  • Performs any other job-related duties as requested.

Requirements

  • Bachelor's degree required.
  • Master's or Doctorate preferred.
  • Three (3) years experience in rehabilitation services or clinical operations required.
  • Previous experience in managed care, home health or long-term care settings preferred.

Qualifications

  • Working knowledge of rehabilitation services and healthcare delivery models.
  • Knowledge of regulatory requirements, service authorization, and documentation standards.
  • Ability to plan, prioritize, and execute work to meet operational goals and deadlines.
  • Ability to analyze information, identify issues, and support problem resolution.
  • Strong written and verbal communication skills.
  • Ability to collaborate effectively across interdisciplinary teams and external providers.
  • Demonstrated accountability for quality, accuracy, and completion of work.
  • Ability to adapt to changing priorities and support process improvements.
  • Ability to incorporate population-specific needs (e.g., geriatric, complex care) into care delivery.

Skills

  • Current, unrestricted Occupational or Physical Therapy license in state of New York required.
  • Valid driver's license, vehicle and verifiable insurance.

Employment Conditions

  • This is a mobile position, meaning that regular travel to different work locations, including homes, offices or other public settings, is essential.
  • Will be exposed to weather conditions typical of the location and may be required to stand and/or sit for long periods of time.
  • Must reside in the same territory they are assigned to work in; exceptions may be considered, due to business need.
  • May be required to travel greater than 50% of time to perform work duties.
  • Required to use general office equipment, such as a telephone, photocopier, fax machine, and computer.
  • Flexible hours, including possible evenings and/or weekends as needed to serve the needs of our members.
  • Over 50% (Mobile) travel may be required to travel to different locations, including homes, offices, or other public settings, to perform work duties.

Compensation

Compensation Range: $72,200.00 - $115,500.00

CareSource takes into consideration a combination of a candidate’s education, training, and experience as well as the position’s scope and complexity, the discretion and latitude required for the role, and other external and internal data when establishing a salary level. In addition to base compensation, you may qualify for a bonus tied to company and individual performance.
