About the role

This is a varied full-time role, primarily based from our office, with time spent out in the community supporting our care teams and customers with complex care needs in their own homes.

Responsibilities

  • Provide clinical oversight for customers with complex care needs
  • Support customers in their own homes through assessments, reviews and clinical guidance
  • Develop, review and monitor care plans and risk assessments
  • Support, coach and develop care teams
  • Complete competency checks, observations and supervisions
  • Support staff training and confidence in complex care
  • Support recruitment, onboarding and induction of new team members
  • Aid with rota planning and service coordination
  • Participate in the shared out-of-hours on-call rota, approximately 1 in 10 weeks
  • Manage customer and staff concerns professionally and compassionately

Requirements

We are looking for a Registered Nurse who is clinically confident, organised and passionate about high-quality care. You may have experience in a hospital, community, care home, complex care, children’s services, mental health or learning disability setting.

Qualifications

  • A valid NMC pin as a Registered Nurse
  • Nursing experience from any clinical setting

  • Strong communication and leadership skills
  • Confidence supporting complex care needs
  • Good organisation and problem-solving skills
  • A commitment to safeguarding, clinical quality and person-centred care
  • The ability to coach, guide and develop care teams
