About the Unit

26-bed Long Term Care Hospital specializing in caring for medically complex patients who require extended hospitalization and advanced clinical care. Typical cases include individuals recovering from critical illness who need ongoing respiratory support, complex wound care, intravenous therapies, cardiac monitoring, dialysis, and comprehensive rehabilitation services before transitioning to a lower level of care.

Additional unit features:

• LEAF system: a wearable patient monitoring technology that helps staff track and document patient repositioning.
• Dialysis treatments at the bedside using TABLO dialysis machines, allowing patients to receive dialysis without leaving their room.

Responsibilities

• Assessment:
  • Conducts individualized patient assessments, prioritizing data collection based on the patient’s immediate condition within specified timeframes.
  • Performs ongoing assessments as determined by patient condition and reprioritizes care accordingly.
  • Integrates nursing interventions into the multidisciplinary plan of care, incorporating standards of care and patient outcomes.
• Planning:
  • Develops, modifies, and integrates an individualized care plan incorporating the medical plan of care, interdisciplinary plans, and standards of practice.
  • Establishes measurable patient goals in consultation with the patient, family, and healthcare team.
• Intervention:
  • Performs treatments as ordered by providers accurately and timely.
  • Delivers therapeutic nursing interventions as established by the individualized plan of care.
  • Recognizes and responds to urgent or emergent risks to the patient, including initiating emergency resuscitative measures.
  • Manages the care of a specialty patient population with responsibility and accountability.
  • Provides culturally sensitive, non-judgmental care, preserving patient autonomy, dignity, and rights.
  • Collaborates with interdisciplinary team members to ensure appropriate patient care interventions.
• Evaluation:
  • Evaluates patient responses to care based on the effectiveness of nursing interventions relative to established goals.
  • Initiates changes in the interdisciplinary care plan based on evaluations.
  • Problem-solves with the specialty team to improve patient outcomes.
  • Documents patient assessment findings, responses to interventions, and progress toward problem resolution.
• Patient Education:
  • Identifies learning needs of patients and families and adapts teaching based on those needs.
  • Ensures patients and families demonstrate knowledge within their abilities.
  • Communicates with patients and families to identify their needs and includes them in the plan of care.
  • Supports patient and family determinations regarding treatment.
• Self and Peer Learning:
  • Assesses and meets own learning and professional development needs.
  • Maintains current competency in nursing specialty and department requirements.
  • Attends required educational programs and reviews professional publications.
  • Seeks additional educational opportunities for clinical development and is prepared to learn or currently functions in a charge nurse role.

  • Makes sound judgments in coordinating increasingly complex patient care demands.
  • Supervises delegated care based on personnel abilities and patient acuity.

  • Serves as a preceptor or mentor and works directly with employees during orientation.
  • May assist in the orientation, instruction, direction, and evaluation of students assigned to the unit.
  • Is trainable for or currently functions in a charge nurse role.

  • Participates in data collection for quality assurance activities if assigned.
  • Identifies and communicates recurring practice concerns in specialty areas and their impact on care.
  • Participates in initiatives resulting from nurse-sensitive, quality, and other specialty indicators or unit-based quality metrics.

• Research/Evidence-Based Practice/Quality Improvement:
  • May participate in research or evidence-based practice projects.
  • Communicates research and quality improvement data/findings to peers.

Requirements

• Diploma or Associate Degree in Nursing.
• Registration as a registered nurse in the Commonwealth of Virginia and/or holds a compact license to practice in Virginia.
• AHA BLS Provider certification.
• Additional specialty certifications required based on unit:
  • Advanced Cardiac Life Support (ACLS) for Cath Lab, Electrophysiology, Operating Room, Critical Care, Endoscopy, PACU, Interventional Radiology, Birth Center, Centra Specialty Hospital, Outpatient Ambulatory Surgery, and Plastic Surgery.
  • Pediatric Advanced Life Support (PALS) for Pediatrics.
  • Both ACLS and PALS for Emergency Department.
• One year of nursing (RN) experience.

Pay

$32.92 - $51.02 per hour.
