Clinical Program Therapist PRN
About the role
The Clinical Program Therapist at Ottumwa Regional Health Center is a licensed behavioral health professional responsible for providing psychiatric care and mental health services to patients across all age groups in Behavioral Health Units. This role requires a Master's Degree in Social Work, Marriage and Family Therapy, or equivalent, with a minimum of 1 year of relevant experience.
Responsibilities
- Conducting assessments and documenting care
- Leading or co-leading therapeutic groups
- Delivering psychoeducational programs
- Cooking discharge planning with patients and their support systems
- Participating within a lead role in the multidisciplinary treatment team planning process
- Encouraging patient and family involvement
- Performing clinical services aimed at stabilizing the psychiatric crisis and developing healthy mental health management through a biopsychosocial model of treatment
- Coordinating discharge plan through discharge planning meetings on assigned units and following up with patients, family, physician, and hospital staff from admission through discharge
- Initiating patient and family education and discharge planning at admission
- Arranging for discharge and post-hospital care of patients through institutions and agencies within the community
- Referring patients and families to appropriate community, social service, home health service, extended care, and rehabilitation facilities, and organizations
- Creating appropriate discharge plans and documenting information in the medical record
Requirements
- Master's Degree in Social Work, Marriage and Family Therapy, or equivalent
- Licenses/Certification: LMSW, LMHC, LCSW, LPC-I, LPC, LMFT
- Independent Practice Scope Licensure in State of Practice preferred
- Minimum 1 year of relevant experience
