About the role

The Minnesota Specialty Health System (MSHS) in Willmar is seeking a Discharge Social Worker to join our team. This position provides referral recommendations and coordinates discharge support, facilitates clients' transition to the community, initiates appropriate referrals for post-discharge follow-up services, ensures outpatient psychiatric appointments are scheduled upon discharge, prepares aftercare plans, and completes provisional discharge paperwork and documentations required by the court.

Responsibilities

  • Coordinates and participates in assigned interdisciplinary treatment team meetings to establish goals for MSHS clients and recommends methods by which these will be attained, including target dates.
  • Facilitates clients’ transition to community by providing information to the client and county case manager or others support as necessary.
  • Initiates appropriate referrals for post-discharge follow-up services.
  • Safeguards outpatient psychiatric appointments scheduled upon discharge.
  • Prepares aftercare plans outlining the client's course through treatment and needs/resources in the community.
  • Completes provisional discharge paperwork and documentations required by the court.

Requirements

  • Possess and maintain a valid driver's license.
  • Bachelor's degree in psychology, social work, behavior analysis, or related field; and Licensure in the State of Minnesota, e.g. LSW or LGSW.
  • One year (1) of post-bachelor's degree experience providing behavioral health services including at least 4 of the 6 activities listed below:
    • Providing direct care and therapeutic support to individuals.
    • Aiding in the assessment of behavior health challenges including data collection.
    • Leading and/or facilitating educational groups or therapeutic activities with individuals with mental health symptoms.
    • Developing or implementing support plans or treatment plans that addresses behavior through adaptations, activities, interventions, education, engagement, and support.
    • Aiding in planning and hand-off communication of support plans and strategies for supporting individuals upon discharge.
    • Developing/assisting in development of behavioral interventions and recommendations for interdisciplinary staff implementation.

Qualifications

  • Master’s degree in psychology, social work, behavior analysis, or related field; and Independent License/Certification in the State of Minnesota (LICSW).
  • Must meet the requirements for a Mental Health Practitioner as defined in MN Statute 245I.04, subdivision 4.

Skills

Supervised training experience obtained as a part of requirements for a bachelor’s degree program may be counted towards years of experience (Pre-graduation experience).

Pay

$28.75 - $42.16 / hourly; $60,030 - $88,030 / annually
